Incident Investigation & Learning

Turn incidents into organisational learning — not just closed actions.

The value of an investigation is the learning it embeds, not the report it produces. Our lead consultant was part of the team that transformed a global operator’s incident-investigation process and has reviewed many high-potential and significant incidents — using Causal Learning and Tripod methodologies to find root causes and the systemic gaps behind them.

What we offer

  • ✓ Major incident investigation
  • ✓ Causal analysis (Causal Learning, Tripod)
  • ✓ Root-cause & systemic-gap identification
  • ✓ Learning-from-incidents systems
  • ✓ Investigation capability building
  • ✓ Investigation process transformation

Frequently asked questions

What makes an incident investigation effective?

An effective investigation looks beyond the immediate cause to the underlying conditions and organisational factors, avoids blame, and produces actions that actually change the system. Success is measured by whether the organisation learns and the same failure does not recur.

What is Tripod?

Tripod is an incident investigation and analysis methodology, originally developed with Shell and academic researchers, that traces an incident back from failed barriers through immediate causes and preconditions to underlying organisational causes.

What is a high-potential incident?

A high-potential incident is an event — often a near miss or minor injury — that could realistically have resulted in a serious injury or fatality under slightly different circumstances. These deserve the same depth of investigation as serious incidents, because they reveal the same weaknesses.

Can you build our internal investigation capability?

Yes. Beyond leading investigations, we train investigators and help redesign investigation and learning-from-incidents processes so your own teams can find systemic causes consistently.
