Human & Organisational Performance (HOP)
Build systems where people can succeed — not programmes that blame them when they don’t.
Most incidents are not a failure of people but of the systems, conditions and organisational factors around them. Human & Organisational Performance (HOP) moves beyond traditional behaviour-based safety to focus on error traps, system conditions, and organisational learning. We help organisations build capacity to learn from normal work — not just from failure. Our lead consultant designed and implemented Crew Resource Management for a global Wells organisation (aligned to IOGP standards), delivered bespoke human-factors and safety leadership programmes to 600+ staff across Malaysia, the UAE, Iraq, Brunei and beyond, and continues to apply HOP principles across every engagement.

Frequently asked questions
What is Human & Organisational Performance (HOP)?
HOP is an approach to safety built on the understanding that people make mistakes, that error is shaped by the systems and conditions around them, and that organisations should learn from normal work rather than only from failure. It focuses on building capacity so that when errors happen, they do not become serious harm.
How is HOP different from behaviour-based safety (BBS)?
Behaviour-based safety concentrates on observing and correcting individual worker behaviour. HOP looks upstream at the error traps, work design, pressures and organisational factors that make unsafe outcomes likely. The two can coexist, but HOP shifts the question from “who got it wrong?” to “what made this make sense at the time?”
What is a learning team?
A learning team is a facilitated session in which the people who do the work explore how it actually gets done — including the workarounds and difficulties — before or after an event. It is one of the most practical HOP tools for finding weaknesses in systems before they cause harm.
What is Crew Resource Management (CRM) and does it apply outside aviation?
CRM is training in non-technical skills such as communication, situational awareness, decision-making, teamwork and leadership. It originated in aviation and is now used in high-hazard industries including oil & gas well operations, where IOGP has published guidance on it.
